Any Oxford University Sports Club who has an accident involving one of it’s members out of Oxfordshire is required to complete an on line Accident Report Form as soon as is possible on the club’s return to Oxford.

To view the explanatory notes on how to complete this form please click here. This applies particularly to cases where the emergency services are called to attend or a member of the club is being detained in hospital for treatment.

Accident Report Forms can be completed on line and sent to the Health and Safety Officer (raegan.shayler@sport.ox.ac.uk) as soon as is practicable. If you wish to add any further information as support, please use an e-mail or send information as a Microsoft Word document.

Accident Report Forms are forwarded to the University Safety Office.
